At Madison School District, our students’ health is very important.  If your child has a health condition such as an Allergy (requiring an EpiPen), Asthma, Diabetes, or a Seizure Disorder, please consider completing the appropriate Health Emergency Care Plan (forms are on the right).  You  may return them to the school secretary or the district office.

If your child needs medication during school hours, please complete the "Medication Administration Request" Form and read Madison School District’s policy regarding medication administration during school hours.

If you have any questions regarding student health management, please fill out the Questions or Comments Information, and the District Nurse will be notified.
